How is the salary worked out?

The calculator runs the full paycheck computation (federal brackets, FICA, state and local tax) and searches for the gross salary whose take-home equals your target — the same maths as a payroll department, run backwards.

Does it include 401(k) or health premiums?

Not by default. Add them in the calculator and the required salary rises to cover them.
